Care Plan for the Dying Person - Victoria
The Victorian End-of-Life Care Coordinating Program (VEC) developed the Care Plan for the Dying Person Victoria (CPDP-Vic) and associated resources with funding provided by the Department of Health and Human Services, Policy and Planning Branch.
This funding ceased in June 2017.
From July 1st 2018 the VEC website will close and Safer Care Victoria will take over responsibility for the CPDP-Vic resources including all future access.
All queries should now be directed to the Safer Care Victoria, Palliative Care Clinicial Network.

As the developers of the CPDP-Vic clinical and educational resources the Centre for Palliative Care is well-placed to develop a customised program of education for your health service.
